SUCCESS THRESHOLD — Continue if 100% of recommendations can be traced back to their supporting inputs and rules, and at least 4/5 can be independently reconstructed well enough for a reviewer to understand and challenge the decision.

A readable explanation is not sufficient: provenance must point to the actual evidence and decision path, not just a post-hoc narrative.
